CredIQ Health
Use this page to request that your CredIQ Health account and associated data are deleted. You do not need to reinstall the app.
This page is for users of CredIQ Health (the CredIQ Health app, Agency Portal, and Clinician Portal). It explains how to request deletion of your CredIQ Health account and the associated personal data we hold. Account deletion permanently removes your login and associated user data; temporarily disabling or freezing an account is not offered as a substitute.
1) Email hello@crediq.health from the email address on your CredIQ Health account. 2) Use the subject line “CredIQ Health Account Deletion Request.” 3) Include your full name, account email, and whether you use the Clinician Portal, Agency Portal, or both. 4) If you have an active paid subscription, cancel it first in your billing settings or ask us to cancel it in the same email. 5) We will verify ownership of the account and confirm when deletion is complete. You can complete this request entirely by email—you do not need to reinstall or open the CredIQ Health app.
Email account deletion requestAfter we verify your request, CredIQ Health deletes account and profile details (such as name, email, phone, organization, and role); authentication credentials; notification preferences and account settings; onboarding and profile information you provided for your own login; documents and credential files stored under your personal account; and technical/usage data tied to your account (for example device or browser identifiers, IP address, and diagnostic logs associated with your user ID).
We may retain limited information when required for legitimate reasons such as security, fraud prevention, tax and accounting, dispute resolution, or other legal/regulatory compliance. Examples include billing and payment records processed via Stripe (we do not store full card numbers), records of the deletion request itself, and anonymized or aggregated analytics that cannot reasonably identify you. Where an agency customer controls clinician or credential records in their CredIQ Health workspace, those business records may remain under that agency’s retention policies (see below).
Personal account data listed under “Data that is deleted” is removed after verification, typically within 30 days. Billing and tax-related records may be retained for up to seven (7) years, or longer if a longer period is required by applicable law. Security, fraud-prevention, and legal hold records are kept only as long as needed for those purposes. Backup copies may persist for a short additional period until rotated out of our backup systems, after which they are permanently removed.
If you are a clinician invited by an agency, your login and personal CredIQ Health account data can be deleted through this process. Credential packets, licenses, onboarding forms, and similar records that the agency stores as part of its compliance operations may remain available to that agency as their business records. Contact your agency for deletion of records they control; we will delete CredIQ-held copies associated with your personal account where CredIQ Health is the controller.
If your CredIQ Health organization has an active paid subscription, cancel the subscription (or ask us to cancel it) before or as part of your deletion request. Deleting an individual user account does not automatically cancel organization billing. Unpaid or disputed charges may still need to be resolved separately.
We aim to complete verified CredIQ Health account deletion requests within 30 days of receiving a complete request. You will receive an email confirmation when processing is finished or if we need more information to verify your identity.
